All athletic paperwork (with the exception of a current physical already on file—see below) is required to be submitted annually. Athletic forms should be turned-in with sports sign-up with the exception of the Parent-Athlete Concussion Awareness Form—state law requires that to be signed AFTER July 1st. Concussion forms will be made available after July 1st and will not be accepted before then.

Student-athletes who do not have the required paperwork submitted prior to splits or practice beginning will NOT be allowed to participate in any on-court activities until everything is submitted.

Paperwork Checklist (see paperwork documents below):

Due to the building renovations, you have 2 options to return your forms:

  1. Return all completed forms & payment in a marked envelope (labeled: Bill Serb, Athletic Director) to the marked box for 2020-2021 Athletics Sign-up box at the St. Joe’s locker pick-up day in early June.
  2. Please complete and mail forms to the school Administration Offices at:

Bill Serb, Athletic Director

Waukesha Catholic School

221 S. Hartwell Avenue

Waukesha, WI 53186

Sports Physicals: State law requires all student-athletes are required to have a medical exam/physical that is signed by a physician. Physicals completed after April 1st are good for the next 2 school years. Physicals prior to April 1st are good for the remainder of that school year and 1 additional year.  As such physicals dated after April 1, 2019 and on-file with the Athletic Director are still good for the coming school year.  Students who do not have a physical on file with the Athletic Director or whose physicals are dated prior to April 1, 2019 will need a new physical on file prior to any participation in sports (including splits AND practice).

If you have any questions, please feel free to reach out.  We look forward to seeing your child participate!

General Information

Waukesha Catholic sponsors three sports - volleyball, basketball and track - for boys and girls in grades five through eight. Teams are separated by grade and gender.

In the fall, Waukesha Catholic teams participate in the Metro Volleyball Conference. Over the winter, Waukesha Catholic teams participate in the Notre Dame/Don Bosco Basketball Conference. In the spring, Waukesha Catholic Track teams participate in the Waukesha Area Catholic Conference.
